created pdf double types

I create PDF's for my agency. I then forward the PDF to the team to use. Most times the document is fillable without issue by the team. A random team member will fill in the form and send it back to us. When we print it off, the typed information the team member inputted is typed 2 times on top of itself creating a blur. Why does this only happen to some and how can I fix this?

Usually a double image is because the field is duplicated with a tiny offset.  But this behavior has been reported without a double field as well. Could also be related to the app that was used to fill the PDF.
