Created PDF fillable form fields disappearing

Question

We have created a PDF fillable form for business purposes that we use repeatedly. However, sometimes when we open the form, some fields do not load. It appears to be random when this will occur as closing and reopening the form several times will eventually get the fields to load. It is always the same fields that do not load when this occurs and we have found no cause of this issue. How do we fix it?
