Creating a dropdown menu for address book.

Dear Community Members,

I want to create a form via Adobe Acrobat DC so I can select a name via a dropdown to fill in the address information. This is the template for a letter, so that I can easily select the correct organization as the addressee. Can anyone help me with the best way to do this?

 

In a way I guess I wanna know if it's possible to give the dropdown item a name, where the visible information placed in the document is much more that just that single name?

 

I hope you guys could help me.
