Creating a multi-level form in a single form file in Adobe Acrobat Standard 9

I am trying to create a form that my front desk may use to enter information. Is there a way to add categories to a drop-down list where if X category gets selected in the drop-down list, a set form appears that is only connected to the X category? Same with any number of categories in the drop-down list. I know how to create a drop-down list, but how do I go about creating a multi-forms in one Form based on the category selected in a drop-down list. Any tutorial, videos, articles to help me with this is greatly appreciated. Please note I use Adobe Acrobat Standard 9 on my office computer and I have Adobe Acrobat DC on my home computer.

Hi,

 

I think you want to look at the template object, as this would allow you to create/show a new page based of the selection in the dropdown.

 

The basic set up would be to have a template for each of the options in the dropdown and then when it is selected you show the template the relates to that option.
