Creating an automatic hyperlink at page level in a PDF - Adobe Pro

My question is: how can I create an automatic hyperlink at page level in a PDF, in Adobe Pro?
Let's say a PDF contains a reference to Document 1, page 50, the reference in the margin becomes DOC-001, p.50.
If I then enable automatic hyperlinking, Adobe Pro only recognizes DOC-001 and not the page number. If I still want to create the hyperlink to page 50, I have to do it manually. How can I ensure that there is automatically a hyperlink to page 50 of DOC-001. In other words, what should that reference look like?
e.g.
DOC-001page50 or DOC-001, p.50 or /DOC-001p50 etc..

Acrobat would not recognize such a text automatically and convert it into a link. It only does that with plain-text URLs. It would have to be added, either manually or using some external tool.

You can link to a specific page of a target file (manually) by creating a link and setting its action to "Go to a page view", then open that file, browse to that page and then click the Set Link button in the floating window.
