Digital signature field not working in PDF Form

I created a PDF Form from an existing PDF file. All of the fields I created seem to work aside from the Digital Signature field, which does not allow the user to sign. The document warnings review reports the following issues.

Report code Description

4000 Unrecognised PDF Content

1001 Comment or form field may silently change

1008 Document contains hidden behavior

1002 Comment or form field may silently change

1008 Document contains hidden behavior

What do these report codes mean? How can I fix them so that the digital signature works?

What happens when you click the field? What application are you using?

When I click on the digital signature field and try to sign with my signature, the Sign button doesn't work. There is a message "Document warnings have been reviewed", and when I click on the Review button, I get a list of document Report codes. Please see screen shots in attached.
