Drawing markups showing up on every page? Why and how to stop it.

Got some documents from a client.  We are using acrobat pro and need to add comments.  Whenever I draw anything using the drawing markup toolbar, it duplicates the rectangle or whatever tool shape I used to the same position on every page of the document.  Any idea why this would happen and how to disable this "feature"?

How strange. Can you post a screenshot of the document with the comments pane open?

To add some more info. It seems to only happen for some of the pages in the document. They are not copied to blank pages in the document, or on a few pages where there are other preexisting comments. There are a few pages where I was able to make comments without them showing up on the rest of the pages, and those pages are also free from the copied comments. I can make new comments on those pages without them showing up elswhere, but only those pages. Also, the comments that show up on a most of the pages were originally made on pages 63 and 64 of the document, but show up as having been made on page 2 (though they still show up throughout the document including page 63-64, but are listed as being from page 2 on the toolbar, ie, if you click it it takes you to page 2, not the page where the comments were originally made). It also does it with other forms of markup, such as highlighting. If i highlight a word on one page, a highlight shows up on every page.
