Drop-down fields based on checkbox

Question

Thank you in advance for any assistance.

I have a checkbox and 3 drop-down list fields associated with it.

When the checkbox is unchecked I'd like for all 3 of the drop-down lists to be hidden.

Upon selection of the checkbox I'd like the first drop-down list to become visible. When I select a dropdown list item in the first drop-down list I'd like for the second dropdown list to become visible. When I select a drop-down list item in the second dropdown list I'd like for the third dropdown list to become visible. The list of choices in each drop-down list is identical, allowing the evaluator to choose up to three separate applicant attributes in a given checkbox category.

If I select the "blank" option in the first drop-down list item (in the case of editing) I'd like for the second and third drop-down lists to return to hidden status.
