Dropdown selection to fill multiple text boxes

Hi All, I am very new to creating custom PDF forms and I am hoping someone can help.

 

I am trying to create a PDF form in which a selection from a dropdown box will fill multiple text boxes with various information.

 

Eg: Select a credit card then pertinent info will be filled into other boxes

Dropdown: Credit Card

Textbox 1: Card Type

Textbox 2: Card Number

etc.

 

I am having trouble figuring out how to setup the dropdown etc. and am not familiar with JavaScript. I am looking for a free option LOL.
