Dynamic file name

Hello,

I have an Acrobat web form that is currently called "Assistance Form." I would like the file name (and email subject line, if possible) to reflect data pulled in from the completed form. For instance, if Jane Doe fills out the web form on 2/10/2025, then it should get emailed as "Assistance Form - Jane Doe 2/10/2025." Is this possible? I have not been able to find documentation in the help pages on this scenario.

Only if you can install a script file on the local computer of each person who is going to be using this form.

Otherwise, you can request they save the file under that name, but not do it yourself.

Setting the subject line of the email is possible, though.
