e-signatures form differences

Hi, 

We're a bit confused. All users are on the latest version of Adobe Acrobat but the e-signature form is different for different people and we cannot understand why.   We have just written procedural documents and the e-signature is different.

One version is looks like this which is what they all want:

For others it looks like this which is not what we want:

Why is there a difference?
