Editable module PDF

Hello community, I would like to create a form with this PDF, where in the second part of the page, if I click on the icon, for example, Mono, it will display the corresponding code '1' in the CODE section above, and similarly for the other values like Beam, Installation, and CCT, ultimately composing the final code. I’ve tried using Adobe forms, but the functionality is too limited. Any suggestions on how I can do this? Thank you.
