Editable PDF issue

Hi. I'm trying to turn a word document into an editable PDF. I'm able to add all the required text fields in using the Prepare Form option , see here: 

....but when I preview the document, you can't see the fields that need to be filled in see here: 

This obviously means that if I send the document as it is, people won't know what they are supposed to fill in and where!

Any idea what I am doing wrong?
