Editing PDF for Patient Interaction and to Generate Barcode

Hello!

 

I've tried contacting a couple businesses about getting help with this but hitting dead ends. Currently, my lab has a requisition form for samples that has to be printed off, marked up, signed, and faxed back to our lab. In an effort to reduce paper and error, I would like to edit the PDF to make all lines either a check box or fillable text.

 

Then, once all of the information is filled, signed, sent over, etc. I'd really like to be able to generate a barcode based off the data in the requisition form. Does anyone have experience with this that would be able to point me in the right direction?

I do, but there are a lot of questions that need to be answered first, such as what barcode methodology you want to use, what the value of the barcode should be based on, etc. Also, the barcode needs to be populated before you fax the file, as doing that will "flatten" it and remove all the fields from it, making it much (much) more difficult to read the data from it in a reliable manner.
