Email Submit Button Pulling Subject Line and Body Message from Form Fields

Looking for a JavaScript to use in a Form Button that will email the PDF file to a hard coded address, pull info from the Form fields to be used for the Subject Line and the Email Body message.  

Any help appreciated.
