Embed a document into a PDF

Please see below image - I have made a word.doc with embedded links, and when I convert to PDF the links don't work. Can someone please tell me how to do this so that when I share the PDF with a client, they can access all the embedded documents by clicking on the links - and not needing access to 'our' systems and folder. Thank you.

How are you converting the files, exactly?

Does your Word document contain the documents? I would just leave the link text in the Word document, and then embed the files in Acrobat into the PDF file, and as a last step, would then add the links to the embedded files.
