Hello,

When trying to add tags to this specific PDF, I get the following message: Adobe Acrobat was unable to make this document accessible because of the following error.

Knowledge source failed. <Clean Up Objects> [511]

Any ideas on how to go about fixing this?

Hi jbcidi

Apologies for the delay in response, I hope you would be able to fix it by now.

If the issue still persists, as described above, you are getting the error knowledge source failed, correct?

What is the dot version of the application, To check the version of the application please refer to the link - Identify the product and its version for Acrobat and Reader DC

Please check for any missing updates. Go to Help > Check for Updates. To know more about the latest version available please refer to the link  - DC Release Notes — Release Notes for Acrobat DC Products

Try to repair the installation (Windows only) Go to Help > Repair Installation

Please try to recreate the PDF file, Go to file and do Save As Adobe PDF.

If the issue is with a particular file could you please share the file with us through the private message for testing purpose How Do I Send  Private Message | Adobe Community

you may also refer to the help article in the link - Create and verify PDF accessibility, Acrobat Pro

Let us know if that works for you
