Error message when Preparing Form

What is the file (customer survey 7) you are starting with?  For example, a Word document?

Sorry to see that you are struggling with the same issue again.

Looking at the screenshot shared above, it seems you are creating form from one of the PDF saved locally on your computer, is that correct? If not, then try saving the PDF locally first and then select "Prepare Form" to continue.

To troubleshoot and get over from this error, you may perform these steps-

• First, Open Adobe Acrobat>Help>Repair Installation> Close Acrobat>Reopen PDF>Prepare form.
• If first solution doesn't work, then you need to run cleaner tool Download Adobe Reader and Acrobat Cleaner Tool - Adobe Labs to remove application and left overs and reinstall it: Download and install Acrobat DC subscription
