field alignment in preview differs from actual pdf form

In the file, everything lines up nicely and the form field widths are set correctly to 4 inches.

The preview looks good.

The saved PDF, when opened, has some fields centered and shortened.

Files attached.

What happened and how do I fix?

I am creating the form directly in Acrobat.

Maybe it's something simple that I'm just overlooking because I've been working nonstop on this (and a few other forms) for 5 hours nonstop!

Looks good in Adobe Reader. We will be putting it on website for clients to download and they won't always be using Adobe. How do I set this up so it will be OK no matter what app they use?


It is a issue of Apple Preview.

Preview on MacOS.

Do I need to save it as Reader Extended?

Open the form in Acrobat Reader or Adobe Acrobat.
