Fill and Sign forms come back empty.

I have a form (Word doc) that has multiple (about 40) fields that I made "fillable" using Acrobat DC.  I wanted to send this form out for signature (1 party) using the Fill and Sign workflow.  I open F&S, add the signature block, and then send the form to the email address of the recipient.  The recipient gets the form, completes all the "fillable" fields, and finalizes the doc with his signature.  The form is returned to me (email) as expected BUT, all the pre-defined "fillable" fields are blank, except for the date field and signature field added with F&S.

I want to save this form (with fillable fields) for repeated use, simply recalling it and running it through F&S to set up the signature field for different individuals.  I do not want to have to set up all the fillable fields using F&S each time I want to send it out.   What am I doing wrong?

We are sorry for the trouble. As described, the form is returning with the blank predefined fillable fields

 

Is this a behavior with a particular PDF form or with all the PDF forms that you send for signature? Please try to send the PDF file to yourself for testing and fill and sign and check for the issue. If its a file specific issue please share the file with us for testing. Upload the file to the document cloud (https://documentcloud.adobe.com/link/home/) generate the link and share it with us.

 

Also, what is the application the users are using to fill and sign the document? what are the steps performed by the users? Please ask them to use the Adobe Acrobat Reader DC  application.

 

You may also try to save the file as Reader extended and see if that makes any difference. Go to File > Save as other > Reader extended PDF  and share the PDF file again with the users to fill and sign and check.

 

What is the version of the Adobe Acrobat DC you are using? To check the version go to Help > About Adobe Acrobat DC

A new version (20.009.20063) is now available for Adobe Acrobat/Reader DC. Please update the application to the new version go to Help > Check for Updates and see if that helps.

 

You may also try to turn off the protected mode for testing and see if that works, Go to Edit (Windows), Adobe Acrobat DC (Mac) > Preferences > Enhanced Security > Turn off the protected mode > Uncheck the Enhanced Security
Note: Please turn on the security after testing.

 

As you want to use the form again for the different individuals, please try creating the template of the form for repetitive use as described in the help article  (https://helpx.adobe.com/in/document-cloud/help/create-template.html)
