Fill in form based on user selection

I have Acrobat Pro DC and I'm trying to see if there's a way to create a form with the following functionality:

1) User enter's an ID number that references a person.

2) The form auto-fill's the name, address, city, state, zip that is referenced by the number after they hit the "lookup button" or something that triggers the lookup.

I could build something like that if the dataset is small, but I'm working with a list of 1000+ people, so, I was hoping there'd be a way to do a lookup into a database or excel spreadsheet based on ID and then return the necessary fields to fill in the form's address, city, state, and zip fields.

Is there any functionality like that? I did a cursory google search and wasn't able to find anything.
