Fill out PDF form in browser

I have made a pdf form for a teaching program. Once the topic has been taught, the teacher checks the "checkbox". The Principal would like form online either in Canvas, Sharepoint of shared on One Drive so she can view what the class is up to. I have upload the form (which only has checkboxes) but I can't work out how it can be "checked off" in the browser and then saved. Is this possible?
