Fillable Form

Ive created a Form with Fillable fields and have saved it so the reader can fill in the slots and sign where the signature field is. however when i senfd the PDF they dont get the fillable spots, they just get the plain form.

Sounds like you may not have either not saved the Fillable Form, or it was saved to a different name than the one you sent.

Can you still open the one that has the fillable fields on it?

Did you test the form with Acrobat Reader?

I just did and the file is a plain form, the fillable spots are not there. Here is the attached form. im trying to tie all the fields to "yes" check box up top
