FILLABLE FORM NOT WORKING ON OTHER COMPUTERS

I have created a fillable form and the date text box and the phone number formatted box does not work. 

The date box is just a regular box and the formatted phone number box is the same. How to I make this permanent?

Additionally, how do I make date box display today's date so it doesn't not have to be entered?
