Fillable forms that generate QR code based on user inputs

I would like to create a form with fillable fields, along with a button that when clicked, will generate a QR code that contains the data filled up by the user.  The form should be able to be filled up by users using Adobe Reader.

May I know which products should I be looking at for this?  I have been doing research and I seem to get pointed to AEM, but information is still very scarce on exactly what is required and what license I need to purchase, etc.
