Fillable PDF returned blank from customer

I created a fillable PDF in Acrobat Pro for customer feedback/input. I have been receiving these with blank fields now and cannot see any customer input in any field.

 

I have a paid subscription to create forms and this was not an issue when using a competitor PDF platform.

 

What is happening?
