Form buttons are showing blank and fillable, but when posted online they show filled out

I received a form to be posted on sharepoint. When I download and open, it looks fine like this:

But the moment, I post it on sharepoint and when opened, it shows like this:

Document was created by someone else on a Mac on InDesign.

We are stumped as we have not come across this before.
