We don't really know. We can only see what works and what doesn't, using trial-and-error.

It is possible to add fields more reliably using a script, though, instead of relying on the Fields Detection Wizard. The problem with scripts, though, is that they can't use graphical elements on the page, only text.

Here are two (paid-for) examples of such tools I've developed for creating text fields (based on underscores) and radio-buttons/check-boxes (based on specific Wingdings characters), that work with 100% accuracy:

When you convert an existing document into a PDF form, Acrobat automatically adds interactive form fields to the form. You can then edit the form to add specialized form fields, such as a drop-down list, list box, or buttons
