Form fields disappear once security is applied

I have form fields in a PDF and did an Encrypt with Password and once I saved the PDF, the form fields disappeared. What am I missing so that I can have my form fields and have a password to open and restrict editing and printing?

    Did you allow form filling when you set your security options?

    In the directions for security it said that users could still fill out form fields with security settings so I didn't look for a specific checkbox to allow that. Can you tell me where that would be?
