Form Fields have come back empty on 2 occasions in the same week

Hi,

I send a questionnaire form to my clients with a lot of fields that need to be completed and then signed. I have been using for over a year sending to multiple clients a month and not had any issues but in the last week 2 forms have come back signed but all of the fields were completely empty apart from their name and the signature and I cannot work out why?

The clients have told me they completed the forms so is there a bug in the system or an issue with adobe saving the information submitted?

This issue may happen if your client isn't using Adobe application for example Reader or Acrobat to fill and sign the form.

Also, let us know how are you sending this form to your client. You may consider using Acrobat sign.
