Form fields not showing on dynamic stamp

Hi all,

I have attempted to create a dynamic stamp following these instructions however i can't get the form fields to show when placing the stamp. Another site said to ensure the stamp is deleted from either the program or user location however this did not resolve the issue.

The only script I'm seeing is on the field on the first page, which you shouldn't use. This is a "cover" page. The stamps start from page 2, and there's where you need to place your code.
