Please confirm if the checked mark disappeared once the document was signed. Also, let us know if these checks are marked by the recipient or prefilled by you.

Did you experience this behavior with the particular document? It would be helpful to have more information on how the document was sent and signed.

This issue is solved when "Use the latest version of Request e-signatures" in setting is unchecked.
