Formatting of form fields vanishes after sending for e-signature

When I send my form for e-signature, the currency formatting in the form fields is lost. The dollar sign vanishes, and it drops the decimal places if they are zeros. How do I fix it? Thank you!

    The Format setting relies on a script, and scripts don't work in Adobe Sign forms. Adobe should make this clearer to people using that application, I think...
