Forms central images

When I insert a image in Forms Central and save in PDF the image is omitted (the space is there). How do i got the image to show up in PDF

FormsCentral has been discontinued a long time ago. Are you still use the standalone app? If so, you do so at your own risk.

I am using Acrobat XI pro and Form Central came with it. I only us the app a few time a year.
