Having a problem with duplicate text when I fill out my form. How do I remove this?

Here is an example of what it is doing. It also prints out that way and I don't know how to fix this. I've searched through forums and all the settings on the text form fields and cannot for the life of me figure out why it is creating this ghost text.

You have duplicate fields one on top of another. If you have Acrobat you can fix this issue by deleting the duplicates.

If you have Reader report this issue to the authors of the form and ask them to fix it and send you a new version.

Thank you! I'm not sure how that happened. But for me it was easier to create an entirely different form than to go in and delete all the duplicates. Again, thank you.
