Header not showing when sending for signatures

Question

I have a header at the top of all my documents with important information for my clients to see. When I send them my files in PDF from adobe with "request e-signatures" it sends the document without the header to be signed. Is there a way to fix this?

Please share the screenshot of the document you uploaded and the document received at the signer's end.

The document should upload completely. How are you creating the document?
