Help creating fillable PDF over background layer

Hello!

 

I am trying to work on a project for a client. Basically - they'd like me to create a PDF version of a form they use that would require a dot matrix printer as it will have three layers (two carbon copies). Currently they use these forms with a typewriter but are looking to move away from using them. My thought was to import the current form as a background layer and lay the fillable boxes above it, so when they print they can hide the background layer and just print the text.

 

Is this even possible? Any ideas? Thank you for any insights. 🙂

You can also use a non printable layer.

You can create a layered PDF with Illustrator, InDesign, Scribus…

I think I figured out how to make sure the background form/image doesn't print but I can't seem to find a way to add a fillable form field without converting it using "prepare a form" which then adds the background layer which I don't want to happen. Is there no option just to add a fillable area on a blank PDF?

Use "Prepare form" to add form fields. Then when you go to print, look at the bottom of the print dialog.  There's a dropdown there labeled "Comments & Forms".  One of the options is "Form Fields Only" select it.
