HELP! tagging is deleting content

Has anyone had any issues when the try to update a tag, or manually tag content, that the content is then erased. This is causing me a huge headache and you can't seem to undo the action.

 

I have never had this issue working with documents that were scanned into pdf, but I am now working on documents that are born digital (i.e., created in some type of word processing then converted to pdf) and am encountering a lot of issues.

    When this happens to me the vanished content is not actually gone, just hiding behind another content element. I open the Content pane and move the hidden content element to be after the hiding content element in the container structure.
