Help with script for interactive form!!

Hi, I'm new at this and need help with the Java script for an interactive subscription PDF Form. The situation is, if the user selects 5 products the amount is X, and the amount is different for all 16 markets. I have added a screen shot for a better understand. So I need Package and sub-total price selected based on market selection. Please help!
