How can i create a link to an fillable form with digital signatures required

I have created a fillable form which requires multiple digital signatures. It is an enrollment packet. I need to be able to create a link from my website that will route visitors to the form to complete. I have several other documents housed on my google drive. I tried to house this form there, but users are not able to fill it or sign.

You need to instruct them to download the file and then open it locally,

using Adobe Reader.

Opening it inside a browser window is not likely to work.

I have been able to improvise by adding a checking box and authorizing an electronic signature. I added a submit button to email once the user has completed the form, however, it is not sending. Is there something in particular I need to successfully accomplish this?
