How can I save filled out PDF form to a jpeg without losing the filled information?

Question

I created a PDF that can be filled out and saved it as "Save as Others / Reader Extended PDF / Enable More Tools (includes form fill in & save)". However, when I filled the form out, save it as another name in PDF and export as jpeg, the information I filled out goes blank. How can I save/export to jpeg without losing the filled out information? Thanks in advance!
