How do I add a "save as" prompt to a form?

I have created several forms used by multiple users.  They are saved in a central location on our server and used repeatedly.  How can I prevent the original blank form from being saved over by a user?  It is frustrating to have to clear the form and then go back and repopulate specific items in the form when someone inadvertently saves over the blank.  How do/can I add a "save as" only feature to the form to make it more like a template?

On the server set the file and folder as read only.

You could add a 'Save As' button in Acrobat.

With the PDF open go to 'Rich Media' select 'Add Button' click and drag to create the button. Then right click to get properties, lable the button and then go to 'Actions' select trigger 'Mouse Down' Select Action 'Execute a menu item' click 'Add' select 'File>Save As...' press ok, then save.

**** Acrobat DC Pro is needed, also you will need to style the button within the properties menu.
