How do I convert a pdf to a fillable form?

I scanned some forms for our science fair, then edited them and now I need to make them into fillable and signable format using Adobe Acrobat DC. How is this done?

Use Prepare Form tool to convert scanned PDF to fillable form. The form fields will be identified.Also, you will be able to add more form fields in the edit mode.

Help - My DC Pro software just stopped enabling me to "select a file" in the prepare form screen. No matter what I click on nothing opens! It worked 2 days ago. HELP....I need this for work purposes!
