How do I create a new form not from an existing document??

I am trying to create a fillable form. I see how to do it if I want to modify an existing document. How do I do it if I want to create it from scratch?

You can create a form from scratch from Prepare Form app, using a blank PDF (via Create PDF app). 1. Goto Tools > Create PDF > Blank Page 2. Goto Tools > Prepare Form > Click Start 3.
