How do I create a new form not from an existing document??

I am trying to create a fillable form.  I see how to do it if I want to modify an existing document.  How do I do it if I want to create it from scratch?

You can create a form from scratch from Prepare Form app, using a blank PDF (via Create PDF app). 1. Goto Tools > Create PDF > Blank Page 2. Goto Tools > Prepare Form > Click Start 3. You will be asked to "Save" the file 4. You can add new form fields on the PDF in Form Edit Mode. For more, you can read https://helpx.adobe.com/acrobat/11/using/creating-distributing-pdf-forms.html

Take a empty PDF, open it in Adobe Acrobat, and use Tools > Prepare Forms
