How do I make a signed and locked document

Hi,

I have a PDF that I want to protect with a signed locked feature. However I cant seem to figure out how to do this. Please find an example I added in this tread. I saw this on another document and tried to figure out how to add this to my own pdf. Looking forward to any ideas, tutorials. Thanks in advance.
