How do you quickly add multiple check boxes in a 3 column form that all align and are the same size?

I have a pdf that I want to add check boxes to which has 3 columns of check boxes and multiple lines. How do I add them quickly and make them all aligned and the same size? See attached PDF.

Select your row of checkboxes, right-click and then select to create multiple copies:

This will bring up a new dialog and you can then specify how exactly you want to create the multiple copies:

Now that I have copied the check boxes, when I check 1 off multiple boxes check off. How do I fix this?

Is there a way to change the field type from text to a check box?

Thank you. When I do this it then randomly creates them and I have to select each one and move it with the arrow keys which takes forever. Is there a faster way to algin them?
