How to add a new column to pdf

I have a pdf template I downloaded and put all my information into. However, it has 5 columns at the bottom and I need to add a 6th column. If you look at the attached pdf, down at the bottom, the first column says “vaccine administered”. I need to resize the 5 columns and add a new column all the way to left that is titled “Admin Date”. How can I do this? Thanks for any help.

    Why in the heck, and how in the heck has Acrobat become a layout tool???  This is the bane of my existence. I send a client a PDF as final, months later it comes back with minor updates, unbeknownst to me, they EDITED type and more, I no longer have the correct NATIVE version.

    Always, always work in the native layout program!!! 

    I hate that Adobe made it this easy to screw up PDF’s…  they used to be final.

    You might want to give detailed steps on how you did it. (I know it could be done a couple of different ways, but I felt the OP would have an easier time in Word.)

    There’s not much to it — just shorten the fields to the desired width, copy the lines from another vertical column, add the new fields, and adjust the title text.

    It would be easier if you get the original Word document. 

    If not, another option is if you have the Creative Cloud that includes Acrobat and InDesign. You may be able to open the PDF in InDesign.
