How to add an existing field to a fillable form

I am working with a fillable form in Adobe Acrobat and a few text fields and a radio button field are in my list of fields (in the right window) but not on my form.  I cannot figure out how to add these fields which have already been defined onto my form.  I cannot drag and drop them from the field list.  When I click Text Box or Radio Button it creates a new field.  How do I put a field that has been defined onto my fillable form?

Delete them via the Fields List and then add them again.

So I ended up deleting and recreating all my radio buttons.  Everything looked good in preview so I saved it out to PDF. I used the Share button to email myself a copy of the form to try it out.   Now, all the new radio buttons appear black even before I have selected either one - and it won't let me select them.  What did I do wrong now and what is the fix?

What happens when you select a field in the field list?

It scrolls to page 3 of the form, but nothing is highlighted.  I don't see the field anywhere on page 3.
