How to create link to automatically fill in a text/number field with pre-defined value

I have a form for evaluating marketing materials. I would like to build this form so that the client can check off on one of the predefined answers and a value associated will appear in the SCORE areas.

As example; if the client checks the box for "No headline at all" box, a value of "0" will appear as the SCORE.

Is this possible in Acrobat Pro 10.1.16 for Mac?

Yes. If those check boxes in the group are intended to be mutually exclusive, it's a bit easier to set up, but you will have more flexibility using JavaScript. If you provide more information, including the names of the check boxes (or radio buttons if you change to using them), as well as what should happen when the other items are selected, we can provide more guidance.
