How to generate customised fillable PDF forms

Hi - I'd like a tool that can generate multiple fillable forms from a single source or template, subject to selecting variables.  At its simplest, it would create multiple forms that are identical except they have different logos and titles.  Ideally however, it could create unique forms with different fields, based on selecting variables.  Any recommendations?

You can try Adobe's Live Cycle Designer.

Such a thing would be possible in Acrobat, but it would also likely involve a good deal of custom programming. The lack of responses may have to do with the complexity of such an approach.
